Where do i complain about an unusual direct debit to homecare insurance. because i never set it up?

Homecare insurance hav been taking money from my account via direct debit from credit union account. which is via th co-operative bank. started at �8.99 on feb 11th and went up to �13.89 every fortnight. Yet co-operative bank and credit union cant agree who is to blame.
